INSTRUCTIONS for becoming a Club Member
Annual dues - .30 per member with a minimum fee of $25 and a maximum fee of $100.
Example:
75 members x .30 = $22.50 and would round up to $25
400 members x .30 = $120 and would round down to $100
Choose "club" as the type of membership below. Please tell us how many members your club had at the end of the previous year. After completing the form, click submit and you will be transferred automatically to Paypal. Enter the payment amount into the "Price Per Item" field and continue to follow prompts. After submitting payment, you will be transferred back to our website.

2020 Full Club Membership Application and Renewal (PDF)
This form is to apply for a membership only. Licensing agreements for your organization's activities must be completed separately. In order to come under our licensing umbrella with the PRO's, please proceed to our Participation Agreement for Weekly, Bi-Weekly or Monthly Parties/Dances/Classes. If your club also hosts major multi-day events and/or conventions, you will also need to fill out a Participation Agreement for Major Multi-Day Events and Conventions for each event.

Annual Dues - $60.00
Choose "supporting" as the type of membership below. Supporting membership is for everyone, except social dance clubs. After completing the form, click submit and you will automatically be transferred to Paypal. After submitting payment, you will be transferred back to our website. This form is to apply for a membership only. One supporting membership form covers all events, no need to fill out a separate membership form for each of your events. Licensing agreements for your organization's activities must be completed separately. In order to come under our licensing umbrella with the PRO's, please proceed to our Participation Agreement for Major Multi-Day Events and Conventions (each event requires a separate form), or Participation Agreement for Parties/Dances/Classes.
